This 3-month MOC improvement project is intended for our internal Primary Care clinicians.

Learning Objectives
Following this improvement project, participants will:
- Define personal patient population that is due, overdue and coming due for vaccinations
- Analyze personal patient population to identify areas of improvement opportunity
- Adopt best practices utilizing the tools and resources identified in the MOC presentations and linked resources
- Report improvement in immunization rates amongst patient population pertinent to personal practice
- Report increased comfort in holding conversations with patient/families with vaccine hesitancies
Steps to Earn and Claim CME Credit:
- Watch Improving Childhood and Adolescent Immunization Rates WebEx in CMEcentral
- Reflect on your current practice and practice data identifying barriers to improvement
- Throughout the project, review patient data and identify and apply strategies to reduce barriers
- Reflect on your project results
- Complete the MOC - PI CME evaluation, attestation and credit verification in CMEcentral by November 26, 2025.
